(2) Complete the registration form below & pay $165 for your class. (NOTE: If you're paying for a group of people, you can choose to call or email us for a group invoice.)
(3) Study your materials & be prepared on training day.
ALSO AVAILABLE: ALCOHOL CERTIFICATION $50 EA WITH MINIMUM OF 10 PER CLASS.

Complete the form below to register online at least 10 days prior to class to receive your study materials in time to prepare for the exam. THE BOOK IS REQUIRED. If you already have the book, there will be a $30 discount. Otherwise, you will receive the book at class.
*You must be pre-registered in order to take the exam. **It is vital the attendee's email is provided in the form below in order to receive the study guide prior to exam!!
